To scuba dive Blue Grotto you must bring us a completed and signed waiver. Be sure to initial the boxes at the bottom of the form. Be aware that, in so doing, you affirm that you watched the Blue Grotto orientation video all the way through.

You can open, complete and even digitally sign this waiver in Adobe Acrobat. Doing so helps ensure your waiver is clear and legible.

Be aware that simply filling out the waiver when it appears on your screen is not enough. You must also download the waiver, print it out, then bring the printed copy with you to the Grotto.

If you are taking part in a class or group activity, give the completed and signed waiver to your instructor or group leader. Otherwise, turn it in when you check in at the store.
