To dive the Grotto you must first complete and sign our standard liability release. If under 18. the waiver must be completed and signed by a parent or legal guardian.
Do this now rather than wasting time doing it when you arrive. This is more considerate of others as they won’t have to wait while you fill out your waiver. It is also important that you complete and sign the waiver before investing any time, money or effort driving to the Grotto. We further want to see that you had plenty of time to reflect on the rights you give up by signing the waiver.
If you are viewing this page on a phone or tablet, stop now and switch to a desktop or laptop computer. You will need the larger screen, a real keyboard and a trackpad or mouse to complete the process.
While you could print this form and fill it out by hand, we prefer you don’t. We want you to do so in Adobe Acrobat instead. Not only will the form be more legible, but you can also sign the waiver digitally, saving both time and effort.

Remember:
- Do not attempt to complete the waiver while it is open in a web browser. The data you enter may be lost and you will not be able to sign the form digitally. Instead, save the form to your computer and re-open it in Adobe Acrobat Pro or Acrobat Reader.
- Be sure to initial the boxes at the bottom of the form. Be aware that, in so doing, you affirm that you have watched the Blue Grotto orientation video all the way through.
When finished, print at least one copy of your completed and digitally signed waiver with you to the Grotto. If coming with your instructor, email them a copy as well.
